29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about October 6, 2016, Aspen Roofing Inc., as controlling employer, did not ensure that employees of Arturo Zavala, subcontractor, were safe from health and safety hazards. No inspections of the job site were conducted by a competent person to ensure that hazards were identified and mitigated. Hazards at the site included falls from the roof and ladder, and potential eye injuries. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
