(a) 211 New Goff Mountain Road:  On or about October 12, 2016 and times prior a worker was observed inside the bucket of a F450 Versalift Truck welding on a metal restaurant sign approximately 22ft. from the ground without the use of a personal fall protection system.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) 211 New Goff Mountain Road:  On or about October 12, 2016 and times prior a worker was observed welding on a metal restaurant sign approximately 22ft. to the ground below without a fall protection system to protect him from falling.
