29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Bldg 8, James H Quillen VAMC- On or about 10/11/16, employees were exposed to electrical hazards when the ground prong was missing from the plug of a portable band saw. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
