29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined on the basis of the following consideration: Heating effects under conditions of use. On or about October 6, 2016, at 1531 Bienville Street, New Orleans, LA, the employer failed to ensure that an electrical cord set that was found to be damaged was taken out of service. Employees were allowed to utilize an extension cord that had visual indications of internal damage as a power source for a Dewalt circular saw.
