29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards shall be guarded with a standard railing and toe board. On or about October 8, 2016, the employer did not ensure maintenance employees working from a work platform above the phosphate plating line chemical tanks were protected from falling into or from the tanks by a standard guardrail and toe board or other form of fall protection.

General-duty citation text
29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. On or about October 8, 2016, the employer did not require maintenance employees to use personal protective equipment including, but not limited to; chemical protective gloves, chemical protective apron, chemical protective boots, face shield, and/or a full body chemical protective suit when working above and around the phosphate line plating chemical tanks. Maintenance employees were exposed to chemicals including, but not limited to, a heated alkaline cleaner, pickling oil, and hydrochloric acid.
