29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug were not visually inspected before use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage, and equipment found damaged or defective was not removed from use until repaired: Automotive garage: On or about October 13, 2016 and at times prior thereto; an extension cord supplying power to a parts washer was missing the grounding prong. This condition exposed the employees to electrical shock hazards.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gasses in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965: Automotive garage: On or about October 13, 2016 and at times prior thereto, one nitrogen cylinders was being stored next to an automotive lift and was not secured from physical damage. This condition exposed the employees to struck-by hazards.
