29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: a) On or about 10/10/2016, at the establishment - North Boundary Road, Building 79, Jamaica, NY A forklift operator raised a load on an angle which caused the pallet to break and fall on the hand of another employee while removing cargo netting from the load. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
