(b) Sales floor: On or about October 13, 2016 and at times prior, employees were exposed to struck by hazards while withdrawing cylinder content from and working in and around one helium bottle missing the valve protection cap while not connected to a regulator or manifold. (c) Storage room: On or about October 13, 2016 and at times prior, employees were exposed to struck by hazards while stacking freight in and around approximately five unsecured helium bottles.
