29 CFR 1926.20(b)(2):      Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On 10/06/2016, sub-contractor employees were working on the roofs of two residential buildings located in the Heritage of Hawk Ridge Subdivision in Lake St. Louis, MO.  Pulte Home, the general contractor or controlling employee, did not conduct an adequate inspection of the job site, materials, and equipment.  Employees were exposed to the following:     1.  On lot 55, 244 Pigeon Drive, employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The foreman and an employee were observed on the roof of the building approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.    2.  On lot 55, 244 Pigeon Drive, employees used a ladder to access the roof which was not extended three feet beyond the landing or edge of the roof    3.  On lot 528 located on Owls Perch Drive, employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The foreman and an employee were observed on the roof of the building approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.
