29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.     Located within Sequence 17 Music Wing on October 10 and/or 11, 2016 Table A type short span open web steel bar joist series 22K7-J240 and 22K9-J237 both exceeding 40 foot 7 inches span were erected with crane supported load lines releasing joist load prior to being welded on both sides of the seat of one end of each steel joist and bolted diagonal erection bridging being installed and anchored without the competent person maintaining performance of inspections on bar joist series type and spans based on Table A of 1926.757 and Partial Roof Joist Erection Plan J2.21 to determine and ensure during the erection, attachment open web steel joist required open web steel bar joist seat welding and bolted erection bridging connections were in place prior to release of load lines.

Located within Sequence 17 Music Wing on October 11, 2016 during the construction period the employer allowed metal decking construction load bundle(s) distribution to be placed on 1926.757 Table A covered type short span open web steel bar joist series 22K7-J240 exceeding 40 foot 7 inches spans were erected without ensuring all erection and horizontal bridging had been installed and anchored and all joist-bearing ends secured with welded attachment anchorage exposing employees to the hazard of being struck by unintentional displacement of open web steel joist which exceeded the carrying capacity of the steel joist within pocketed bearing points.
