29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end. (a) At the 4545 W. Pine St., Unit K, Appleton, WI jobsite; the scaffold pick installed on the scaffold extended more than 12 inches past the upright exposing employees to a tipping hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) At the 4545 W. Pine St., Unit K, Appleton, WI jobsite; the employer did not train employees on the proper use and erection of the scaffold. The employees were working from a platform made up of scaffold plank, scaffold board, plywood, and 2x10 boards not secured together. This exposed employee(s) to tipping hazards due to its construction.
