29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    a)  Employees were exposed to electric shock/ burns.  The employer did not inspect of the work site where employees were going to work in an energized panel to ensure proper means and methods were followed.  An arc flash occurred when the employee was working with the wiring in the panel.    Location: 388 Greenwich St, New York, N.Y.,  40th fl mechanical rm.      On or about 10/08/2016        NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

Employees were not instructed on the means, methods and pre-task review to work on energized panels (480VAC) when connecting wiring in panels DPH-40B-2 and 40-B-4.  An arc flash occurred when the employee was working in the panel.    Location: 388 Greenwich St, New York, N.Y.,  40th fl mechanical rm.      On or about 10/08/2016        NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
