29 CFR 1926.501(b)(4)(i): Each employee on a walking/working surfaces was not protected from falling through holes(including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about October 13, 2016 and at times prior to that date, for the employees exposed to an approximate 10 foot fall hazard after accessing the floor hatch to performing work activities in the east penthouse room no.1000 located at 510 Eighth Avenue NE, Hazen, North Dakota. (b) On or about October 13, 2016 and at times prior to that date, for the employees exposed to an approximate 10 foot fall hazard after accessing the floor hatch to performing work activities in the west penthouse room no.1001 located at 510 Eighth Avenue NE, Hazen, North Dakota.
