29 CFR 1910.39(c)(1): A fire prevention plan did not include, a list of all major fire hazards, proper handling and storage procedures for hazardous materials, potential ignition sources and their control, and the type of fire protection equipment necessary to control each major hazard:     a) This violation was most recently observed on or about October 14, 2016 and times prior thereto, the facility did not list the gas fired boilers in the north refrigeration room as potential ignition sources exposing employees to hazards associated with fire/explosion.      b) This violation was most recently observed on or about October 14, 2016 and times prior thereto, the facility did not list the gas fired boiler in the room adjacent to the north refrigeration room as a potential ignition source exposing employees to hazards associated with fire/explosion.

29 CFR 1910.119(d)(3)(iii): For existing equipment designed and constructed in accordance with codes, standards, or practices no longer in general use, the employer did not determine and document that the equipment in the process was designed, maintained, inspected, tested, and operating in a safe manner:    a) This violation was most recently observed on or about October 14, 2016 and times prior thereto, the north refrigeration room at OK Foods located in Oklahoma City, Oklahoma contains gas-fired boilers in the same area as the ammonia refrigeration equipment.    b) This violation was most recently observed on or about October 14, 2016 and times prior thereto, the ammonia detectors in the north refrigeration room at OK Foods located in Oklahoma City, Oklahoma were not operable.

29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the PHA team's findings and recommendations:     a) This violation was most recently observed on or about October 14, 2016 and times prior thereto, in the north ammonia refrigeration room at OK Foods located in Oklahoma City, Oklahoma. The employer did not assure that 2015 Process Hazard Analysis recommendations such as, but not limited to, ensuring ammonia detectors were operable were resolved in a timely manner exposing employees to the hazard of contact with/inhalation of anhydrous ammonia.       b) This violation was most recently observed on or about October 14, 2016 and times prior thereto, in the north ammonia refrigeration room at OK Foods located in Oklahoma City, Oklahoma. The employer did not assure that 2015 Process Hazard Analysis recommendations such as, but not limited to, conducting emergency mock drills were resolved in a timely manner exposing employees to the hazard of contact with/inhalation of anhydrous ammonia.      c) This violation was most recently observed on or about October 14, 2016 and times prior thereto, in the north ammonia refrigeration room at OK Foods located in Oklahoma City, Oklahoma. The employer did not assure that 2016 Process Hazard Analysis recommendations such as, but not limited to, conducting emergency mock drills were resolved in a timely manner exposing employees to the hazard of contact with/inhalation of anhydrous ammonia.
