OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to the employee, in that employee were exposed to the hazard of being struck and/or hit by falling metal stock from the overhead cranes:   under hung hoists in Bays 1 and 2:      ESTABLISHMENT:  The employer did not ensure that the employees, operating the No. 1, No. 2, and No. 3 overhead cranes under hung hoists, were protected from the hazards of being struck and/or hit by falling metal stock all times due to lack of deficiencies, such as (but not limited to) pendant control not marked "on/off", no bumpers, no capacity marking for slings and hooks.

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     WORKSHOP: The employer had not established written step-by-step energy control procedures in the machine specific for the employees during servicing and maintenance of equipment, such as (but not limited to) the overhead cranes, Birch sheet roller, and Hdracrop-100-SD iron worker.      ABATED DURING THE INSPECTION.

29 CFR 1910.179(e)(2)(i): A crane shall be provided with bumpers or other automatic means providing equivalent effect, unless the crane travels at a slow rate of speed and has a faster deceleration rate due to the use of sleeve bearings, or is not operated near the ends of bridge and trolley travel, or is restricted to a limited distance by the nature of the crane operation and there is no hazard of striking any object in this limited distance, or is used in similar operating conditions. The bumpers shall be capable of stopping the crane (not including the lifted load) at an average rate of deceleration not to exceed 3 ft/s/s when traveling in either direction at 20 percent of the rated load speed.    WORKSHOP: The No. 5 and No 6 (marked No. 9) overhead cranes were not provided with bumpers or other equivalent energy absorbing materials/devices.

The double alloy steel chain sling used with the No. 1 overhead crane under hung hoist, unknown identification located in Bay 1 did not have permanently affixed tag.         2.
