OSH ACT of 1970 Section (5)(a)(1): Employee(s) were exposed to the hazard of being struck-by: Employee(s) in building 2 of the Dollar General Fulton, Missouri warehouse were exposed to a struck-by hazard due to working within a long space in between staged pallets of materials where the employees were not visible to power industrial truck operators and the operators were moving pallets. Methods of abatement to correct this apparent hazard could include use spotters or to configure stacks so that operators can see employees working between rows or prohibit working between pallets when operators are moving or handling the pallets or materials.

29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s): Capstone Logistics forklift operators used the practice of bulldozing pallets when making rows of pallets such as in building 2, Dollar General, Fulton, MO, between the warehouse dock doors and fixed racking, including pallets stacked two levels high and without a spotter. Dollar General employees routinely walk in the aisle ways between the rows of pallets potentially being exposed to pallets being moved during the pushing practice. There was no rated capacity for pallets being bulldozed or other method that determined how many pallets could be bulldozed. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
