29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  a) Exterior of Building - Scaffold on Brinkerhoff Facade: Employees were exposed to a fall hazards ranging from approximately 12 feet to 14 feet while engaged in masonry activities from a welded frame scaffold that was resting on hollow blocks, on or about 5/28/16.

Brinkerhoff Ave Facade: Employees without any means of fall protection and engaged in masonry activities installing bricks from a welded frame scaffold platform were exposed to a fall hazards ranging from approximately 12 feet to 14 feet, on or about 5/28/16.
