29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Lot 9, 2nd Floor - on or about October 13, 2016, employees engaged in residential construction activities were accessing the second floor via an extension ladder to an upper landing with an unguarded floor edge, approximately nine feet above the lower level, which was not protected to prevent falling to the lower level. b) Lot 9, Rooftop - on or about October 13, 2016, employees engaged in sheathing a steep sloped rooftop were working on the frame members of the trusses, greater than six feet above lower levels, without utilizing any means of fall protection. c) Lot 9, North West Side (Front) - on or about October 13, 2016, employees engaged in residential construction activities were working on the second floor near three windows openings, which were not guarded to protect employees from falling to lower levels greater than six feet. d) Lot 9, South West Side ( Right) - on or about October 13, 2016, employees engaged in residential construction activities were working on the second floor near three window openings, which were not guarded to protect employee from falling to lower levels greater than six feet. e) Lot 9, South East Side (Back) - on or about October 13, 2016, employees engaged in residential construction activities were working on the second floor near three window openings, which were not guarded to protect employees from falling to lower levels greater than six feet. f) Lot 9, North East Side (Left) - on or about October 13, 2016, employees engaged in residential construction activities were working on the second floor near one window with an opening, which was not guarded to protect employees from falling to lower levels greater than six feet.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) Lot 9, Rooftop - on or about October 13, 2016, employees climbed from the second story onto the top plate frame members to access the rooftop to perform residential construction activities, exposing employees to fall hazards greater than six feet to lower levels.
