29 CFR 1926.251(a)(2)(i): Employers did not ensure that rigging equipment had permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load;   (a) Jobsite - On or about October 14, 2016 chains used to lift concrete lift station sections did not have identification tags to indicate the safe working load.

29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:   (a) Jobsite - On or about October 14, 2016 employee not protected from falls when working to install concrete lift station in an open trench. The employee was exposed to a fall height of 7 feet.        i. - Cut edge of trench used as a walkway       ii. - Bucket of Volvo EC 240c track hoe used as a work platform

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that:   (a) Jobsite - On or about October 14, 2016 employee was working in an excavation that was greater than 5 feet deep. Employee working in type B soil was not protected from cave-in hazard by a trench shield protective system or sloping and benching system.

29 CFR 1926.652(c)(4)(iii): At least one copy of the design shall be maintained at the jobsite during construction of the protective system. After that time, the design may be stored off the jobsite, but a copy of the design shall be made available to the Secretary upon request.   (a) Jobsite - On or about October 14, 2016 employer did not have design information for a ridged trench box used in a Class B soil trench.
