On October 17, 2016, on the top roof, employees nailing plywood sheathing were not protected from a 13 feet fall hazard by the use of a fall protection system.     b. On October 17, 2016, on the middle roof, employees handling plywood were not protected from an 10 feet fall hazard by the use of a fall protection system.     c. On October 17, 2016, on the lower roof, employees handling plywood were not protected from an 18 feet fall hazard by the use of a fall protection system.

On October 17, 2016, the ladder used to access the 3rd level did not extend 3 feet above the landing, exposing employees to an 8 feet fall hazard.    b. On October 17, 2016, the ladder used to access the 4th level did not extend 3 feet above the landing, exposing employees to an 8 feet fall hazard.    c. On October 17, 2016, the ladder used to access the top level roof did not extend 3 feet above the landing, exposing employees to a 13 feet fall hazard.
