29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. On 10/8/16, at the State Farm Perimeter job site, employees working from 8' A-Frame ladders were not required to barricade the area around the ladder when working just outside an unsecured doorway.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. On 10/9/16, the employer did not establish a ladder training program for employees using ladders to prevent injury when using ladder equipment.
