On or about October 5, 2016, on the peak of the roof of the commissary, the Hoar Construction Safety Manager and electrician employees of Consolidated Electrical Contractors & Engineers, Inc. were exposed to a 30 feet fall hazard from the roof in that they were not protected by a fall protection system.  b.  On or about October 5, 2016, the employer did not erect guard rails or provide anchor points for personal fall protection at the unprotected edge above the meat manager office, exposing employees of Hoar Construction, electrician employees of Consolidated Electrical Contractors and Engineers, Inc., and employees of Mohr Services to a 10 feet fall hazard from the top of the cooler to the office below.

On or about October 5, 2016, at the NAS Jacksonville Commissary above the meat, produce, frozen foods, and dairy coolers and freezers, Hoar Construction employees, a Mohr Service employee installing refrigeration controls, and electrician employees of Consolidated Electrical Contractors & Engineers were exposed to trip hazards from 6 to 12 inch-wide floor holes between the top edges of the coolers and the surrounding walls in that covers had not been placed over the holes.
