29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 18 October 2016, and at times prior thereto, four framers were working on a home under construction with overhead hazards without the use of hard hats, exposing the workers to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 18 October 2016, and at times prior thereto, two workers were using pneumatic air guns to applying Thermo Ply sheets to a residence under construction without the use of safety glasses, exposing the workers to the hazard of being struck by flying objects or projectiles.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about 18 October 2016, and at times prior thereto, a worker was standing on the ladder platform of a pump jack scaffold at a height above 10 feet without any type of fall protection, exposing the worker to a fall hazard of approximately 13 feet 6 inches to the ground below.

On or about 18 October 2016, and at times prior thereto, a framer was working from a pump jack scaffold system without the three pole legs being tied into the structure with metal brackets, exposing the worker to a fall hazard of approximately 13 feet 6 inches to the ground below.
