29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Southern States, LLC, Hampton, GA- Employees are required to drill (lathe) a fiberglass tube into smaller pieces with a lathe machine.  This fiberglass powder creates a dusty environment that causes exposures to the employees face, eyes, skin and body.  A bulk sample of the powdered product revealed traces of benzene, ethyl benzene, xylene. o-cymene and fiberglass, which are skin irritants.   The employer had not accessed the workplace, trained employees on personal protective equipment, and did not ensure that employees removed contaminated clothing/footwear and washed their skin immediately with soap and water after exposure, on or about 10/17/2016.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:   Southern States, LLC, Hampton, GA- Employees are required to drill (lathe) a fiberglass tube into smaller pieces with a lathe machine.  This fiberglass powder creates a dusty environment that cause exposures to the employees face, eyes, skin and body.  A bulk sample of the powdered product revealed traces of benzene, ethyl benzene, xylene. o-cymene and fiberglass, which are skin irritants.   The employer had not identified and evaluated the respiratory hazard(s) to which the workers are exposed, on or about 10/17/2016.  The employer had not conducted monitoring.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  Southern States, LLC, Hampton, GA- Employees are required to drill (lathe)  a fiberglass tube into smaller pieces with a lathe machine.  This fiberglass powder creates a dusty environment that causes exposures to the employees face, eyes, skin and body.  A bulk sample of the powdered product revealed traces of benzene, ethyl benzene, xylene. o-cymene and fiberglass, which are skin irritants.   Employee(s) were required to drill the fiberglass tube while not being cleaned shaven, on or about 10/17/2016.  Employees had symptoms of upper respiratory irritation.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
