Business Dr. Sheboygan, WI: On and before October 18, 2016, employees had been exposed to falls while working from scaffolding that was decked with 2x10 lumber that had cracks and rotten areas which would compromise the strength of the planks.

29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platform more than 2 feet above or below a point of access:  Torres Masonry, LLC project site located at 6710 S. Business Dr. Sheboygan, WI: On and before October 18, 2016, employees were exposed to falls while accessing the working levels of a scaffold when no ladder was provided for safe access and egress.  The employees were climbing the 6'-9" pass through scaffold frames which are not intended to be used as ladders.  Torres Masonry, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 998229, citation number 1, item number 1A, and was affirmed as a final order on 11/21/14, with respect to a workplace located at 1331 N. 377th, Roanoke, TX 76262.

Business Dr. Sheboygan, WI: On and before October 18, 2016, employees were exposed to falls while installing EIFS on a scaffold that did not have guardrails where employees were walking and working.  Torres Masonry, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 998229, citation number 1, item number 2, and was affirmed as a final order on 11/21/14, with respect to a workplace located at 1331 N. 377th, Roanoke, TX 76262.

Business Dr. Sheboygan, WI: On and before October 18, 2016, employees were exposed to fall hazards while working from scaffolding when they had not been trained to properly erect the scaffold or to recognize and correct hazards related to the scaffold, to include but not limited to, proper planking, acceptable access, guardrails, and inspection.   Torres Masonry, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 998229, citation number 1, item number 3, and was affirmed as a final order on 11/21/14, with respect to a workplace located at 1331 N. 377th, Roanoke, TX 76262.
