29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.  a. Tencom Ltd. located in Holland, OH:  On or about October 9, 2016, the employer did not ensure an employee had received training on when to verify zero energy on electrical systems and what electrical protective equipment needed to be worn when working on electrical systems.  The employee did not verified with test equipment that the electrical limit switch on the Kalamazoo saw, located in building#2, was deenergized prior disconnecting the switch from the electrical system and the employee was not provided with the proper electrical protective equipment when working on the electrical system.

The specific safety-related work practices were not consistent with the nature and extent of the associated electrical hazards.  a. Tencom Ltd. located in Holland, OH:  On or about October 9, 2016, the employer did not ensure safety related work practices were followed when an employee failed to verify zero energy on an electrical system and did not wear electrical protective equipment prior to performing work on the 110 volt electrical interlock for the Kalamazoo saw located in building #2.

29 CFR 1910.333(b)(2)(iv)(B): A qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees were exposed and did not verify that the circuit elements and equipment parts were deenergized. The test did not determine if any energized condition existed as a result of inadvertently induced voltage or unrelated voltage backfeed even though specific parts of the circuit had been deenergized and presumed to be safe. If the circuit to be tested is over 600 volts, nominal, the test equipment shall be checked for proper operation immediately after this test.  a. Tencom Ltd. located in Holland, OH:  On or about October 9, 2016, the employer did not ensure an employee verified with test equipment that the electrical limit switch on the Kalamazoo saw located in building#2 was deenergized prior disconnecting the switch from the electrical system and installing a new switch.
