29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI: Custom Welding, Inc. project site located at 3265 Cty Hwy AB, Luxemburg, WI: On and before October 17, 2016, employees were exposed to falls and struck by hazards while using a Genie S-40 boom lift as a work platform and material hoist which had been modified by attaching a synthetic sling to the midrail of the platform.
