29 CFR 1910.106(b)(2)(vii)(a): Area(s) surrounding tank(s) containing flammable liquids were not provided with adequate drainage or dikes: a) 244 Old Hwy 149, Cumberland City, TN- On or about 11/01/16, above ground fuel tanks on site did not have any drainage or diking or collision barriers to prevent spillage of the diesel fuel. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. (a) Tennessee Valley Authority - Cumberland City Fossil Plant - On or about 10/13/2016 at 1:30 a.m., an employee suffered a partial amputation of the end of his left index finger and the employer did not report the amputation to OSHA until 3:15 p.m. on 10/18/2016. IN ACCORDANCE WITH 29 CFR 1903.19(C), ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION (USING THE CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
