On or about October 19, 2016, at the above addressed jobsite, employees performing carpentry operations on the second floor roof, were exposed to falls of approximately fourteen (14) feet five (5) inches due to lack of fall protection.   b.  On or about October 19, 2016, at the above addressed jobsite, employees performing carpentry operations on the second floor, were exposed to a fall of approximately fourteen (14) feet five (5) inches created by the presence of window openings.
