29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about October 13, 2016, the employer did not ensure employees were utilizing the procedures for the safe application, usage, and removal of potentially hazardous energy controls while clearing jammed parts at the #19 Auto Tapper Machine air blower and chain conveyer machine area. Energy sources include, but not limited to; electric, pneumatic, and/or hydraulic power.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about October 13, 2016, the employer did not ensure a tapping machine operator clearing jammed parts at the #19 Auto Tapper Machine air blower and chain conveyer was trained in the methods and means necessary for energy isolation and control. Energy sources include, but not limited to; electric, pneumatic, and/or hydraulic power.
