29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about October 19, 2016, employees were exposed to being stuck by flying objects.  The employees were framing an addition on an existing residential home located in Wadsworth, Ohio 44281.  The employees were using hand saws and air powered nail guns without wearing safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about October 19, 2016, employees were exposed to a fall of approximately 20 feet.  The employees were framing an addition on an existing residential home located in Wadsworth, Ohio 44281.  The employees were performing sheathing activities on the roof without using any form of fall protection.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.      On or about October 19, 2016, employees were exposed to a fall of approximately 12 feet.  The employees were framing an addition on an existing residential home located in Wadsworth, Ohio 44281.  There were three window openings on the front of the new structure.  The window openings did not have a guardrail installed.  The employees were working in front of the window openings without any form of fall protection.
