29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Defense Logistics Agency: Cheatham Annex: On or about 10/04/2016, an employee was hospitalized as a result of an incident that occurred in the workplace. On 10/12/2016, the employer reported the inpatient hospitalization to the Department of Labor in Norfolk Virginia. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
