29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: (a) McGee Construction LLC at 6700 S. Colebrook Cir., Sioux Falls, SD 57108: On or about October 18, 2016 the employer did not provide a training program for each employee using ladders to train each employee in the procedures to be followed to minimize hazards related to ladders. This condition exposed the employees to a fall hazard of up to approximately 10 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)McGee Construction LLC. @ 6700 S Colebrook Cir., Sioux Falls, SD 57108: On or about October 18, 2016, employees were exposed to injuries from falls, while conducting residential framing activities in excess of six feet above the lower working surface, without the benefit of fall protection systems being in place. This condition exposed employees to fall hazards of approximately 10 feet. McGee Construction LLC. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 962489, Citation Number 1, Item Number 1, and was affirmed as a final order on May 13, 2014 with respect to a workplace located north of Bitterroot Street West, Sioux Falls, SD 57108.
