29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Northwest Corner of Building - On or about October 18, 2016, two employees were working from an aerial lift (e.g. self-propelled telescopic boom) without the use of personal fall protection or restraint devices, thus exposing employees to a fall hazard of about 13 feet, 4 inches onto concrete and asphalt ground.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) Parking Lot (East of Building) - On or about October 18, 2016, the outer sheathing on the flexible cord used to energize a DeWalt miter saw was pulled away from its connection point at both ends of the cord, thus exposing employees to electrical hazards.
