29 CFR 1926.25(a):     During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:      a)  8 Venturi Court, Florham Park, NJ.    Employees were exposed to injury from trips and falls and from protruding nails as old siding material was allowed to accumulate in the immediate work area around the pump jack scaffolding.                            Violation observed on or about 10/18/2016.

29 CFR 1926.451(b)(5)(ii):     The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:    a)  8 Venturi Court, Florham Park, NJ.    Employees were exposed to falls of up to 12 feet, 10 inches to the ground below from pump jack scaffolding as the 2 ends of the platform extended over their supports by 4 and 5 feet lengths with no end guardrails blocking employees from accessing the cantilevered end.                            Violation observed on or about 10/18/2016.

29 CFR 1926.451(f)(3):     Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a)  8 Venturi Court, Florham Park, NJ.    Employees were exposed to falls of up to 12 feet, 10 inches to the ground below from pump jack scaffolding as  a competent person had not inspected the scaffold or its components for defects as shown by the 2 ends of the platform extending over their supports by 4 and 5 feet lengths and as shown by the lack of fall protection for employees working on the scaffolding.                            Violation observed on or about 10/18/2016.

An approximate 20 foot extension ladder was used as the pump jack scaffold's work rest which is not the intended use for a ladder and neither the scaffold manufacturer nor any ladder manufacture would approve this type of use for a ladder.                                       Violation observed on or about 08/30/2016.
