29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:         (a)  Avedon Engineering, Inc. at 811 S Sherman St., Longmont CO 80501:  On and before October 18, 2016, the employer did not ensure that employees were protected from injury, in that maintenance employees used a scissor lift, which was positioned at the exterior of the building, to access the roof.  The employees did not utilize personal fall arrest systems after they exited the lift at the roof level and for the entirety they worked on the roof.  This condition exposed employees to a fall of approximately 20 feet.

Maintenance employees used a scissor lift, which was positioned at the exterior of the building, to access the roof. This condition exposed employees to a fall of approximately 20 feet.    Abatement Note:  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that shall include the following:      1)     The workplace evaluated;  2)     The person certifying that the evaluation had been performed;  3)     The date(s) of the hazard assessment; and,  4)     The document identified as a certification of the hazard assessment.
