29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:         The employer is failing to protect employees from fall hazards associated with roofing without adequate fall protection.  The most recent occurrence of this was observed at 5417 S 62nd St., Lincoln NE 68516 where employees were observed working on a roof that was approximately 10' high and they were not using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    The employer is failing to protect employees from a fall hazard associated with a ladder not extending at least 3 feet above the upper landing surface. This was most recently observed at 5417 S 62nd St, Lincoln, NE, 68516, where employees were observed utilizing an extension ladder to ascend an approximately 10' high roof and the ladder only extended approximately 2 feet above the upper landing surface.

The most recent occurrence of this was observed at 5417 S 62nd St., Lincoln NE 68516 where an extension ladder was taken apart and one half was being used to ascend a roof approximately 10' high.
