29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Location:  239 Witmoor Forrest Court, Weldon Spring, MO  63304  At the job site, a worker was not wearing a protective helmet while exposed to struck-by hazards.  The worker was picking up debris on the ground directly below the eave where the roofing crew working on the residential roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Workers were exposed to a fall hazard as they were actively conducting roofing activities on an approximate 8:12 pitch roof without any form of fall protection.  The eave to ground fall distance was approximately 15 feet, 4 inches at the SE corner and 13 feet, 9 inches at the NE corner of the residence located at 239 Witmoor Forest Court, Weldon Spring, MO  63304.
