29 CFR 1926.1404(d)(1): Before commencing assembly/disassembly operations, the A/D director did not ensure that the crew members understand all of the following (I)-(iii): a) On or about October 18, 2016 at Westrock, 145 Highway 155 South, Cottonton, Al: The employer did not ensure the A/D Director reviewed with the crew members the hazard associated with disassembly of the fly boom jib by instructing the single man to ensure he had visual/voice confirmation of the employee removing pins was in a safe position before giving the crane operator the hand signal retract telescoping boom.
