29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Shop Area - On or about October 17, 2016, the employer did not ensure that the clearance between the table and the sanding disc was maintained at 1/16" of an inch on the PSA Disc Sander, catalog #ISG-00035-D. An employee using the disc sander to de-burr metal parts experience an amputation injury when his thumb was caught between the table and the sanding disc.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: a) Shop Area - On or about October 17, 2016, the employer did not ensure that the flexible cord on the PSA Disc Sander, catalog #ISG-00035-D, was provided with adequate strain relief.
