29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a) Graley Auto Body: On October 20, 2016 and times prior, employees were exposed to a fall of 13 feet 5 inches from the roof to the ground below while installing metal roof panels on a roof with a slope of 6:12. Employees were not protected from falling from the roof to the ground below by a guardrail system or personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Graley Auto Body: On October 20, 2016 and times prior, employees were exposed to a fall of 13 feet 5 inches from the roof to the ground below while installing metal roof panels on a roof with a slope of 6:12. The employer has not provided employees with training on recognizing fall hazards and the procedures required to be protected from the fall hazard.
