Building 1 Export Office, where outlet AX-4 was equipped with a cracked and loose faceplate.  2. Exit Booth, where the outlet located beneath the counter was equipped with a cracked faceplate exposing insulated conductors.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:  a) Facility, where employees were exposed to tripping hazards due to damaged floor coverings including, but not limited to:   1. Building 1 Locker Area, where a tear in the carpeting located in the aisle measured approximately 93 inches long.   2. Building 1 Locker Area, where a tear in the carpeting located in the aisle way measured 12 inches long.   3. Building 1 ATU Office, where a tear in the carpeting located in the aisle way measured approximately 52 inches long.  4. Building 2 Third Floor, where a tear in the carpeting located in the aisle way adjacent to the Copy Room measured approximately 26 inches long.  5. Building 2 Third Floor Conference Room, where a tear in the carpeting located in the aisle way measured approximately 31 inches long.   6. Building 2 Third Floor Conference Room, where a tear in the carpeting located in the doorway measured approximately 3 inches long.   7. Building 2 Third Floor Hallway, where a tear in the carpeting located in the aisle way adjacent to the filing cabinets measured approximately 35 inches long.  8. Building 2 Third Floor Property Room, where a tear in the carpeting located in the aisle way measured approximately 50 inches long.  9. Building 2 Second Floor Team 795 Cubicles, where a tear in the carpeting located beneath the rolling  office chairs measured approximately 60 inches long.  10. Building 2 First Floor Training Office, where a tear in the carpeting located in the aisle way measured approximately 13 inches long.  11. Building 2 First Floor Cargo Office, where a tear in the carpeting located in the aisle way measured approximately 5 inches long.  12. Booth 6, where the floor tiles had become uneven due to wear. Building 2 CAS Office, where a relocatable power tap was used to supply power to appliances including the Magic Chef microwave oven, Keurig coffee maker, and the refrigerator.   5. Building 2 Copy Room, where a relocatable power tap was used to supply power to appliances including the Cuisinart coffee maker.   6. Building 2 Office 301, where a relocatable power tap was used to supply power to appliances including the GE microwave oven.   7. Building 2 Intel Office, where a relocatable power tap was used to supply power to appliances including the Pelonis portable floor heater. (Item Corrected)  8. Booth 1, east side door was equipped with an inoperable lock.  2. Booth 3, east door frame was bent and the door was not equipped with a lock.   3. Booth 4, the west window was not equipped with a lock.   4. Booth 5, equipped with damaged door frames, and the east door was not equipped with a lock.
