The scaffold was not properly braced in that there was no bracing at the bottom of the poles.

Supervisors' (including foremen) safety and health responsibilities understood. C.  Employees adherence to safety rules.    3.  Identification and Control of Hazards A. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Right Side of home: On or about October 20, 2016, an employee was exposed to a fall of up to 16 feet while using a Werner extension ladder to access the platform of a pump jack scaffold to help install the fascia on the roof edge.  The ladder was not extended 3 feet above the edge of the platform.
