On October 20, 2016, employees working in the Hickory Point Lift Station excavation were exposed to struck by hazards from the soil and rock on the north side of the shoring system after the shoring was pushed down and the remaining soil and rocks were not protected with a suitable means.    b. On October 20, 2016, employees working in the Hickory Point Lift Station excavation were exposed to hazards after a large steel plate was welded onto the southeast side of the shoring system to prevent soil from entering from the bottom of the excavation and the addition was not approved by a registered professional engineer.
