29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which would prevent pull from being directly transmitted to joints or terminal screws: a) 666 Bliss Rd., Longmeadow, MA: On or about October 14, 2016 employees were using an orange extension cord, to power various tools, that had its outer jacket pulled out from the plug housing, exposing the internal wiring of the extension cord.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) 666 Bliss Rd., Longmeadow, MA: On or about October 14, 2016 an employee working on the roof of the commercial property being built at this site was exposed to falls in excess of 18 feet to the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
