29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) On the roof, on or about or at times prior to October 19, 2016, employees performing roofing work with pneumatic nail guns were not utilizing eye protection, resulting in an eye injury hazard.  The Mario Salgado Chavela dba Graciela Reza was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910 102(a)(1), which was contained in OSHA inspection number 1111104, citation number 1, item number 1 and was affirmed as a final order on May 6, 2015 with respect to a workplace located at 165 Sailmaker Lane, (Lot #133) and 45 Glen Way (Lot#113) in Richmond Hill, GA 31324.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On the roof, on or about and at times prior to October 19, 2016, employees performing roofing work were not utilizing fall protection, resulting in an 18.7-foot fall hazard.  The Mario Salgado Chavela dba Graciela Reza was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910 501(b)(13), which was contained in OSHA inspection number 1111104, citation number 1, item number 2 and was affirmed as a final order on May 6, 2015 with respect to a workplace located at 165 Sailmaker Lane, (Lot #133) and 45 Glen Way (Lot#113) in Richmond Hill, GA 31324.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   (a) At the front of the residence, on or about and at times prior to October 19, 2016, the metal extension ladder used to access the roof area did not extend 3 feet beyond the roof line, resulting in a 9-foot fall hazard onto the lower roof.  Mario Salgado Chavela dba Graciela Reza was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1111104, citation number 1, item number 4 and was affirmed as a final order on May 6, 2015, with respect to a workplace located at 165 Sailmaker Lane (Lot#133) and 45 Glen Way (Lot#113) in Richmond Hill, GA.
