OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe lacerations and amputation hazards from unintended activation of the saw blade:    (a) 1201-5000 Frame Department:  On or about October 13, 2016 and times prior, the Norgren foot pedal, used to actuate the 12 inch saw blade on the SC12 Sampson cutoff saw, was not fully guarded.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  (a) 1201-5000 Frame Department: On or about September 23, 2016 and times prior, the Sampson SC12 cutoff saw was not de-energized by disconnecting the electrical power and the airline prior to reaching into the saw through the guard opening to remove vinyl debris and scrap near the point of operation.
