29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: 326 Chapman St, Greenfield, MA On October 19, 2016, two employees were installing moisture barrier membrane and placing shingles on a roof ridge at a height of 16 feet and were not protected from falling by guardrail systems, safety net systems or personal fall arrest systems. On October 19, 2016, an employee was injured after falling sixteen feet from the roof rake to the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 326 Chapman St, Greenfield, MA On October 21, 2016 and at times prior, the employer had not provided fall protection training for two full-time employees whose duties required them to work on roofs with fall hazards. The employer had not provided fall protection training for short-term employees who were exposed to fall hazards while performing roofing work.
