29 CFR 1926.451(f)(6): The clearance between scaffolds and 13,800 volt un-insulated power lines were not kept at a minimum of 10 feet away when scaffolds were erected, used, dismantled, altered, or moved such that they or any conductive material handled on them might come closer to exposed and energized power lines: Construction site: On or about October 14, 2016, and at times thereof, employees performing plastering work on a framed scaffold were exposed to un-insulated electrical power distribution lines (13,800 volt) within 10-feet from the scaffolds. This condition exposed employees to electrical shock hazards.

Construction site: On or about October 14, 2016, employees were using a 20-foot aluminum extension ladder that was not extended at least 3-feet above a roof landing surface. This condition exposed employees to fall hazards greater than 6-feet.
