29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. On or about October 18, 2016, and at times prior thereto, there was a hose running over and across the metal suit case (next to the mud pumps) which was sticking up approximately 3 inches. Employees were exposed to a trip and fall hazard.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: On or about October 18, 2016, and at times prior thereto, in the change room: there was a heater which had the flexible conduit that housed some electrical conductors pulled out from the heater, exposing the insulation on the conductors to damage. Employees were exposed to the hazard of electrical shock.
